As our clinic requires reservations, for those who are visiting for the first time, please don‘t forget to make a reservation by telephone or on our website, and bring your Japanese Health Insurance Card if you have.

Registration

Please bring your health insurance card for every visit.
Our clinic has a priority for patients with reservations which is why there may be a wait if you visit without one. We kindly request you make an appointment in advance.
Patients over 15 years old only.
Please wear a mask during your visit.

NOTE：If you have common cold symptoms such as fever, cough, sore throat, and runny nose, please call in advance to make a reservation.

Please print out and fill out the medical questionnaire in advance and hand it to the receptionist upon your arrival.

Directions from Ginza Station [A5] Exit

  ①After exiting the ticket gate, head towards Ginza Station's A5 exit.

  ②Please go up these stairs.

  ③Once you go up the stairs, you will see the building in which our hospital is located (Yotsuya Gakuin Building).

  ⑥Please take this elevator to the 5th floor.

  ①Exit through the ticket gates marked A2 to A4.
• 
  ②Proceed along the passage towards Showa-dori, Miharabashi intersection, and Higashi-Ginza.

  ③Proceed to Exit "S3".
• 
  ④Exit at "S3".
• 
  ⑤Once you've climbed the stairs, turn left.
• 
  ⑥After climbing the stairs, you will see the entrance to the building in which our hospital is located immediately on your left.
